Monument Health informed of data security issue

RAPID CITY, S.D. — On Friday, Monument Health announced that they were informed of a data breach from one of its vendors, MGC Health LLC, that could affect some of their patients.

MGC determined that an unauthorized party obtained certain personal information about some of their patients. Monument Health says that the information may include names, social security numbers, medical codes, postal addresses, telephone numbers, email addresses, dates of birth and gender.

To assist with the investigation, MGC is coordinating with law enforcement authorities and hired a leading forensic investigation firm.

Patients who have been affected by the breach will be contacted by MGC through mail. MGC also advises that patients monitor their credit reports and account information for unfamiliar activity.

If you think that you may have been affected, MGC has provided a toll-free number that is available on weekdays from 7 a.m. to 9 p.m. and on weekends from 9 a.m. to 6 p.m. The number is 866-475-7221.
